Can't add APN

I have an LG X Power on Android 6.0.1. I just ported over from Boost Mobile today. I can make calls and SMS. I do not have data over network and am trying to follow the instructions to set it up but there is not way for me to add an APN, no "+" and the two APNs that are listed ("ota" and "otasn") are both grayed-out.

I was getting "Restricted access changed" and Data service blocked" message when I first switched but I don't see those messages any longer. Apps are saying "Please enable your Internet connection..." and "net:: ERR_INTERNET_DISCONNECTED"

Under Settings > Network: My Mobile data is on. Network mode is Automatic.
Under More: Mobile data is "On". Roaming is "Use roaming".
Under 4G settings: Network operator is blank. IP address is:
Network mode is "Automatic"
Access Point Names: "ota" and "otasn" are both grayed-out.

Under Settings > General > About Phone: Network is "T-Mobile". Mobile network type is "EDGE". Service State is "In service". Mobile network state is "Disconnected". IP Address is "Unavailable".

I tried updating everything I could think of. I shut down, removed the SIM card, rebooted, shut down, inserted SIM card, rebooted.

Nothing seems to help.
